    Immunizations
    According to state law, immunizations are required for students entering Oregon public schools. These may be obtained from the family doctor, local county health department, or health screening clinic.

    Oregon law states that every child attending any Oregon public, private, or parochial school must have proof of immunization or meet the requirements for exceptions. Children previously immunized must submit, at registration time, a signed statement showing that immunization has been completed against Diphtheria-Whooping Cough-Tetanus (DTaP), Polio, Measles, Mumps and Rubella (MMR) and Haemophilus Influenzae (HIB). In cases involving special health problems or religious beliefs, forms will be available for doctor or parent signature waiving this requirement. Immunization requirements have been changed to reflect the additions of vaccine or exemption for Hepatitis B, Varicella (chickenpox) and second dose Measles.     Emergency Card
    An Emergency Card will be filled out for each student at the beginning of each school year and kept on file for emergency purposes.

    Transcript
    A Transcript Form will be provided for parents to fill out for school records to be transferred from their children's previous school.

    Proof of Age
    Proof of age is required for entering kindergartners and first graders. A child may be admitted to kindergarten if, and only if, his or her 5th birthday occurs on or before September 1. A child entering the first grade may be admitted if, and only if, his or her 6th birthday occurs on or before September 1. If the age of the child is questioned, the principal may demand and receive legal documentation. Admissions can be refused.

    Social Security Number
    The Oregon Department of Education requests that parents provide the Social Security number for their child as part of the enrollment information. Social Security numbers are used on some ODE reports.

    Physical Examination
    The Hood River County School District requests that entering kindergartners and first graders provide evidence of a physical examination prior to being admitted to school.
